Policies & Payment

Payment

Payment is due at the time of service. I accept all major credit cards, HSA/FSA cards, and debit cards through my secure client portal.

Cancellation Policy

Appointments canceled with less than 24 hours’ notice, or missed without notice, will be charged a $50 fee. This policy allows me to reserve time for you and ensures availability for other clients who may need an appointment.

Superbills

I am an out-of-network provider and do not bill insurance directly. However, many clients are able to receive partial reimbursement through their insurance’s out-of-network benefits. Upon request, I can provide a superbill (an itemized receipt) for you to submit to your insurance company.

Sliding Scale Fee

I understand that cost can sometimes be a barrier to accessing therapy. To support clients experiencing financial hardship, I offer a limited number of sliding scale appointments at a reduced fee.

If you are interested in this option, please complete the form below to see if you are eligible for a sliding scale fee discount. Availability is limited and reviewed annually.

Sliding Scale Fee Request

Aluna Therapy offers a limited number of sliding scale fee spots based on financial need and availability. Sliding scale fees are determined using the Federal Poverty Level (FPL) Guidelines, which consider both your household income and family size.

Please fill out the following form

Total Family Size

Enter the number of people in your household who are supported by your income (yourself, spouse/partner, children, and any other dependents). Do not include roommates or others who support themselves.

Enter the combined gross income (before taxes) for everyone in your household who contributes to household expenses. This includes wages, self-employment, benefits, child support, retirement, and investment income.
